VIPT New Patient Forms

Your First Step Toward Feeling Better Starts Here

At VIPT, we understand that starting physical therapy can feel like a big step—especially if you’ve been dealing with pain, stiffness, injury, or limited mobility for some time. Whether your discomfort is new or something you’ve been living with, you don’t have to push through it alone.

Our mission is simple: help you move better, feel stronger, and get back to living your life with confidence.

By completing your New Patient Forms, you’re taking an important step toward recovery—and we’re honored to be part of that journey with you.

Personalized Care, Designed Around You

No two bodies are the same, and neither are recovery journeys. At VIPT, your care begins with truly understanding you—your symptoms, your goals, and how pain or limitations are affecting your daily life.

Your intake forms help us:

  • Understand where you’re feeling discomfort or restriction
  • Learn about past injuries, surgeries, or medical conditions
  • Identify your personal goals—whether that’s returning to work, staying active, avoiding surgery, or simply moving without pain
  • This information allows our licensed physical therapists to create a customized, one-on-one treatment plan tailored specifically to your needs.

A Supportive, Judgment-Free Environment at Home or Clinic

Many patients worry that they’re “too stiff,” “too out of shape,” or that they’ve waited too long to seek care. Please know—you are exactly where you need to be.

At VIPT, we meet you where you are today. Dr. Stanley Ha is here to support, encourage, and guide you at a pace that feels safe and achievable. Progress doesn’t require perfection—just consistency and the right care.
